Clinical Background & Evidence Base

The Ocular Surface Disease Index (OSDI) is a 12-item validated questionnaire assessing dry eye frequency, environmental triggers (wind, air conditioning), and vision-related limitations. Score = (Sum of question scores * 25) / (Number of questions answered). Scores range from 0 to 100, stratified according to the Tear Film & Ocular Surface Society (TFOS DEWS II) consensus.

Clinical Staging Reference

Classification / Stage Clinical Interpretation & Care Pathway
Normal Ocular Surface (OSDI 0 – 12) Healthy tear film stability and lipid layer. Maintain routine eyelid hygiene and hydration.
Mild Dry Eye (OSDI 13 – 22) Episodic irritation or environmental strain. Preservative-free lubricating eye drops (carboxymethylcellulose or sodium hyaluronate) and warm compresses recommended.
Moderate Dry Eye Disease (OSDI 23 – 32) Persistent burning, foreign body sensation, and fluctuant vision. Clinical tear breakup time (TBUT), meibomian gland expression, and lipid-based drops advised.
Severe Ocular Surface Disease (OSDI 33 – 100) Disabling ocular discomfort and corneal epitheliopathy. Requires specialist therapy including topical cyclosporine, punctal plugs, or therapeutic scleral lenses.
